Activation Fee
One time $50 activation fee per member or $100 activation fee per family. (Activation fee is waived if yearly membership paid in full)
Membership Includes:
● 2 Yearly Cleanings ($280 value)
● 2 Yearly Exams ($227 value)
● All Necessary X-rays (Panoramic X-ray $180, Bitewing X-rays $97, Full Mouth Series of X-rays $196. Excludes Cone Beam or 3D imaging.
● 2 yearly Fluoride Treatments for children ($120 value)
● 2 Emergency visits per year ($240 value)
● 10% off ALL dental work, including crowns, whitening, orthodontics, and implant procedures completed in our office.
● No waiting periods, no annual maximums.
If you have dental insurance, we can still submit the claims for you, and you will receive reimbursement checks from your insurance company directly!
